feeeei 3abd646582 fix(ops): Gemini 原生路由的网关本地错误按错误类型归类 
Google 格式错误体没有 type 字段，ops 一律记为 api_error，客户端参数错误、
引用不存在的缓存等都被归为平台内部错误。

- googleError 写出时按状态码向 ops 登记错误类型：400/413 invalid_request_error、
  401 authentication_error、403 permission_error、404 not_found_error，
  429 与 5xx 不登记
- 计费、并发与风控拦截沿用 Claude / OpenAI 格式错误的类型与错误码，归类口径一致
- 存储价未配置、显式缓存未启用改用 googlePlatformError，仍按平台错误统计；
  错误透传规则改写的上游错误不登记本地类型
- 上游返回的缓存不存在标记为请求级失败：按客户端请求错误分类，
  上游原始错误保留为明细但不参与上游归因

feeeei f4f8ff04db fix(gateway): 客户端断开统一标记 499,不再被记成 502/200 上游错误 
客户端在响应提交前断开时,只有排队/换号阶段和 OpenAI 的兜底写入会标 499。
Claude 非流式与 Gemini 的 Chat Completions / Messages / Responses 兼容入口
在等待上游时断开,由 GatewayHandler.ensureForwardErrorResponse 补写 502;
Gemini 原生入口什么都不写,落成 200 并被 ops 记成一条 provider 的
"Recovered upstream error";Antigravity 回写 502。这些行都按上游失败计入
SLA 与上游错误数。

- GatewayHandler.ensureForwardErrorResponse 补客户端断开检查,与 OpenAI 版一致
- GatewayHandler 的 Messages / ChatCompletions / Responses 与 Gemini 原生入口:
  首次选号失败时先判断客户端断开;ChatCompletions / Responses 循环开头改用
  failoverClientGone,不再直接 return 落成 200
- Gemini 原生入口:forward 失败时补 499 标记;并发槽错误改走
  concurrencyErrorResponse 统一映射(断开 499、排队满/超时 429、其余 503)
- Antigravity 上游响应前断开回写 499;googleapi 与 Antigravity 的 Google 错误
  格式把 499 映射为 CANCELLED
- OpenAI alpha search 非 failover 错误分支补断开检查
- Claude / Gemini / OpenAI 传输层错误:请求 context 已取消时不记 ops 上游事件
- ops:IgnoreContextCanceled 开启时跳过没有上游错误上下文的 499;带上游错误的
  499(上游失败后客户端没等到换号结果就离开)仍按上游失败落库

计费不变:Gemini 流式中途断开的用量处理不在本次范围。新增 handler/service
单测覆盖各入口的断开场景与 ops 过滤规则。

feeeei 44bc47a3e3 fix(gemini): 原生路径登记 2xx 响应里的带内错误信号 
Gemini API-key 原生转发对上游 2xx 只抽 usage 原样透传：finishReason 内容拦截、
promptFeedback.blockReason、事件里的 Google 错误信封、空响应都被当作成功，
ops 错误日志、用户错误页、渠道监控全部没有记录。

- 新增 gemini_response_signal：逐事件判定三类带内信号，流式按优先级登记最高者；
  没有 data 事件时对非 SSE 的兜底正文整段判定，空体登记 EMPTY_STREAM / EMPTY_RESPONSE
- 内容策略类按请求级结果登记（invalid_request_error / 400，business limited，
  不做上游归因、不计 SLA）；错误信封、生成异常、空响应按上游失败登记并计 SLA
- OpsStreamError 新增 RequestScoped / NonStream：请求级带内行不受残留上游上下文与
  skip_monitoring 影响，落库状态取 IntendedStatus；中间件保留此前尝试的恢复行
- 客户端字节、wire 状态、用量与计费不变

Eyre921 5aba53d542 fix(ops): 记录固化 200 SSE 流上的就地错误，修复流内限流不进错误看板 
流式请求一旦 flush 了 keepalive ping，HTTP 状态码即固化为 200；此后
出现的错误（等待并发槽位超时后回退的限流、Wait 后二次计费校验失败、
流开始后才无可用账号等）只能就地以 SSE error 帧回传。而 ops_error_logger
以 status>=400 为采集触发条件，这类挂在 200 流上的失败此前会在错误看板里
完全隐形——客户端能收到 rate_limit_error，但网关侧没有任何错误记录可供排障。

- service: 新增 OpsStreamError 上下文 + MarkOpsStreamError/GetOpsStreamError，
  采用「首个标记生效」保留根因错误，避免被后续通用兜底帧覆盖。
- handler: handleStreamingAwareError 在 streamStarted 分支标记流内错误。
- handler: OpsErrorLoggerMiddleware 在 status<400 且无上游错误上下文时，
  据标记补记一条错误日志；分级用 IntendedStatus（如并发限流 429），
  StatusCode 仍记 wire 的 200。上游透传错误已由 upstream-context 分支落库，
  故此路径不重复记录。
- 补充单测覆盖补记、no-op、skip_monitoring 跳过与首个标记生效。

DaydreamCodingandClaude Opus 4.8 b62b573f7f feat(openai): cyber_policy 硬阻断全链路透传、审计与计费 
上游对单次请求下发 error.code=cyber_policy 硬阻断时,网关在所有端点
(/v1/responses、/v1/chat/completions、/v1/messages、WebSocket)及流式/
非流式路径下,将该结果原样透传给客户端,绝不 failover、换号或同步拦截;
命中后异步完成审计与计费:

- 风控中心记录 cyber_policy 留痕并发送通知邮件,落库先于发信,SMTP 阻塞
  不影响留痕
- ops 错误请求记录,状态码对齐客户端实际接收(流式 200 / 非流式 400)
- 用量明细标记 request_type=cyber,按上游真实 token 计费,HTTP 与
  WebSocket 计费口径统一,零 token 命中不误扣
- 会话级自动屏蔽(管理员开关,默认关):命中的会话在可配 TTL 内本地拦截
  不再发往上游,仅屏蔽该会话不影响同 Key 其他会话
- 封号计数排除开关:可选让 cyber 命中不计入自动封号,命中当次不判定且
  历史行在违规计数中一并排除

WebSocket 多轮连接下 cyber 标记按 turn 生命周期管理,逐轮独立检测与记录;
透传的错误响应不被兜底逻辑追加内容污染。

DaydreamCodingandClaude Opus 4.8 ddf063352a feat(ops): 错误日志 key 归因与早退字段补全 
让 /admin/ops 错误详情正确归因 API key 并补全早退场景字段，合并三项改动:

- 鉴权早退补全用户/分组/平台字段:引入 ops fallback key(ContextKeyOpsFallbackAPIKey),
  apiKey 一加载成功即写入,覆盖分组停用/删除、Key 停用/过期/额度、用户停用、IP 限制等早退
  路径;ops 错误日志改用 getOpsAPIKey(正式 key 优先、回退键兜底),不改「已鉴权」语义。
- 已删除 key 归因(迁移 145):删除 key 时同一事务写 deleted_api_key_audits 映射,认证失败
  时用明文反查命中原所有者,错误详情展示「已删除 Key 所有者」「尝试的 Key 前缀」。
- 有效 key 报错快照前缀(迁移 147):对绑定有效 key 的错误,落库时快照明文前 8 位到
  api_key_prefix(与 attempted_key_prefix 互斥),key 之后被删仍保留报错当时真实前缀。

均仅对上线后新产生的错误/删除生效。

alfadbandClaude Opus 4.6 ce006a7a91 fix(ops): validate error_type against known whitelist before classification 
Upstream proxies (account 4, 112) return `"<nil>"` as the error.type in
their JSON responses — a Go fmt.Sprintf("%v", nil) artifact. Since
`normalizeOpsErrorType` only checked for empty string, the literal
"<nil>" passed through and poisoned the entire classification chain:
error_phase was misclassified as "internal" (instead of "request"),
severity was inflated to P2, and the stored error_type was meaningless.

Add `isKnownOpsErrorType` whitelist so any unrecognised type falls
through to the code-based or default "api_error" classification.

Edric Li d95e04fd1f feat: 错误透传规则支持 skip_monitoring 跳过运维监控记录 
在每条错误透传规则上新增 skip_monitoring 选项，开启后匹配该规则的错误
不会被记录到 ops_error_logs，减少监控噪音。默认关闭，不影响现有规则。

Zero Clover ad1cdba338 feat(ops): 支持过滤无效 API Key 错误，不写入错误日志 
新增 IgnoreInvalidApiKeyErrors 开关，启用后 INVALID_API_KEY 和
API_KEY_REQUIRED 错误将被完全跳过，不写入 Ops 错误日志。
这些错误由用户错误配置导致，与服务质量无关。

Zero Clover 673caf41a0 feat(ops): 将 USER_INACTIVE 错误排除在 SLA 统计之外 
将账户停用 (USER_INACTIVE) 导致的请求失败视为业务限制类错误，不计入 SLA 和错误率统计。

账户停用是预期内的业务结果，不应被视为系统错误或服务质量问题。此改动使错误分类更加准确，避免将预期的业务限制误报为系统故障。

修改内容：
- 在 classifyOpsIsBusinessLimited 函数中添加 USER_INACTIVE 错误码
- 该类错误不再触发错误率告警
